Requestline is a show for patients of The Royal Blackburn Hospital to be able to hear
their favourite music. The show is on every night at 8pm, please see our programmes

A collector visits the wards collecting requests. Requests can also be made in many
other ways, filling out this form is one of them.
You can also use this form if you just want to have a message read out
to someone you know. If this is the case then please specify when you want the
message read out and we will do our best to make sure it happens.
